Magnolia Gastroenterology Clinic, LLC charges for office visits with our doctor or nurse practitioner, professional fees for the doctor or
endoscopist
who performs your procedure, professional fees for the anesthesia provider or certified registered nurse anesthetist (CRNA), and some laboratory and pathology professional fees.
It covers professional fees only and does not include facility fees for scheduled procedures. You may receive separate bills for laboratory, pathology, or facility fees.
Most insurance companies reimburse us for medical care. Every attempt will be made to verify your insurance coverage in advance in order to accurately inform you of any other costs. You are responsible for determining if preauthorization
is required by your insurance company.
Bring your current insurance card and photo ID, such as a driver’s license or state-issued photo ID. The clinic will file the insurance claim for you. However, the responsibility for payment rests with you if the insurance company denies payment.
Keep the following information available when you arrive for admission:
- Name of the insurance company
- Employer information
- ID or policy number
- Social Security number of the insured
- Claims address of the insurance company

Magnolia Endoscopy Center, LLC charges a predetermined fee for each type of procedure. This fee covers facility fees only, including your procedure, nursing service, supplies, and medications used during your procedure.
Professional service fees for the doctor, anesthesiologist, lab, or pathology aren’t included. The fees will be billed separately.
